Something along the lines of Fable mechanics where every action or choice you make has a reaction affecting your own character development and the environment around you. Now that would be pretty badass to incorporate in a MMO, unfortunately it also sounds quite difficult to pull off when you got thousands of people per server making similar or different choices for each situation.
