While it's entirely plausible that the Waking the Beast titles are supposed to be exclusive with each other, it would be the only real instance of it in the entire game. There's a lot of quests where you only get one of X titles when doing it, but completing it allows access to all the alternatives as well. That's why I personally consider it a bug.
But I'll try my third completion of it later tonight after work if no one else has done it by then to try and confirm it's still an issue.

Bastok has Xarc/Beaucedine this week but not having any luck unlocking Steel Resolve IV or Hazardous Materials for them. Are there any secondary requirements for them? I unlocked SR II like normal but after throwing another ~40 tags at SR II I've still yet to unlock SR III. Also turned in 2 sets of stronghold boxes to see if that'd help but no go so far.
Campaign ops likely have invisible requirements of nation stats to be available on top of (or perhaps due to) the repetition of previous tiers. These stats will go up over the week and we just reset yesterday. It's not uncommon that some ops take several days to show up, just check in every so often.

Friends and I did around 30 runs of Pit Spider 2 on Windy this week after the restart without Pit Spider 3 opening. I read a note about By Light of Fire needing Recon 5+, so on a lark I pushed Windy's Recon to 4 and Pit Spider 3 immediately appeared.

Got 1100 RoEs complete but no rank up from 8 to 9 still. As expected though.

So these two are still mutually exclusive and the only example of such in the game that I know of. Probably should still be reported.

Following up on tis, we did a bunch of Pit Spider 3 runs (well over 20) without unlocking By Light of Fire. I pushed Recon from 5 to 6 and it immediately showed up.
